5 Early Warning Signs of Stroke You Should Never Ignore

A stroke is a medical emergency. Every minute that passes without treatment can result in the death of millions of brain cells, leading to permanent disability or even death. According to the World Health Organization, stroke is the second leading cause of death globally and a major cause of long-term disability. Recognizing the early warning signs can mean the difference between full recovery and lifelong impairment. Understanding What a Stroke Is

A stroke occurs when blood supply to a part of the brain is suddenly cut off — either due to a clot (ischemic stroke, accounting for about 80% of cases) or a ruptured blood vessel causing bleeding in the brain (hemorrhagic stroke). Without a constant supply of oxygen-rich blood, brain tissue begins to die rapidly.

There is also a condition called a Transient Ischemic Attack (TIA) or “mini-stroke,” where blood flow is temporarily disrupted and symptoms resolve within minutes to hours. TIAs are critical warning signs of an impending full stroke and must be taken equally seriously.

Warning Sign 1: Sudden Numbness or Weakness on One Side of the Body

One of the most classic signs of a stroke is sudden weakness, numbness, or paralysis affecting one side of the body — particularly the face, arm, or leg. The person may notice that one arm drops when both are raised, or one side of their face droops when they smile. This occurs because a stroke typically affects one hemisphere of the brain, which controls the opposite side of the body.

Do not mistake this for tiredness or sleeping on one side. Any new, sudden weakness on one side of the body is a neurological emergency.

Warning Sign 2: Sudden Confusion or Trouble Speaking

A stroke can affect the language and speech centers of the brain, causing sudden confusion, difficulty finding words, slurred speech, or an inability to understand what others are saying. The person may appear intoxicated even though they have not consumed alcohol. They may speak in jumbled words, use wrong words, or be unable to speak at all.

This sign is often mistakenly dismissed or attributed to fatigue. Sudden speech or language difficulty is always a red flag requiring immediate medical attention.

Warning Sign 3: Sudden Vision Problems

A stroke can cause sudden loss of vision in one or both eyes, double vision, or blurred vision. The person may report seeing darkness on one side of their visual field (a condition called homonymous hemianopia). In some cases, they may have difficulty judging distances or perceiving depth.

Visual disturbances in stroke are different from the aura of migraines — they typically come on abruptly, without warning, and do not resolve quickly on their own.

Warning Sign 4: Sudden Severe Headache

A hemorrhagic stroke (bleeding in or around the brain) often presents as a sudden, explosive headache — frequently described as the worst headache of one’s life. This “thunderclap headache” may be accompanied by neck stiffness, vomiting, sensitivity to light, and loss of consciousness.

Unlike a typical migraine, this headache reaches peak intensity almost instantaneously. Anyone experiencing this type of headache should be rushed to the emergency room without delay.

Warning Sign 5: Sudden Loss of Balance or Coordination

A stroke affecting the cerebellum or brainstem can cause sudden dizziness, loss of balance, unsteady walking, or lack of coordination. The person may appear drunk, stumble, or be unable to walk in a straight line. When combined with other stroke symptoms, sudden balance disturbance is a strong indicator of a stroke.

Note that isolated dizziness alone is usually not a stroke — but when it occurs alongside other warning signs such as facial drooping or arm weakness, it should be treated as an emergency.

The FAST Rule — Remember It, Share It

The simplest way to remember stroke warning signs is the globally recognized FAST acronym:

F – Face drooping: Ask the person to smile. Does one side droop? A – Arm weakness: Ask the person to raise both arms. Does one drift downward? S – Speech difficulty: Is speech slurred, garbled, or strange? T – Time to call emergency services: If any of these signs are present, call 112 immediately.

Some experts use an extended version — BE-FAST — that adds: B – Balance (sudden loss) E – Eyes (sudden vision changes)

Every second counts. Do not wait to “see if it gets better.” Get to a hospital with a stroke unit immediately.

What to Do If You Suspect a Stroke

  • Call emergency services (112) immediately
  • Note the exact time symptoms began — this is crucial for treatment decisions
  • Do not give the person food or water
  • Do not let the person “sleep it off”
  • If the person is unconscious, place them in the recovery position

Treatment with clot-busting drugs (tPA) must be given within 4.5 hours of stroke onset, and mechanical thrombectomy can be performed within 24 hours in some cases. Time truly is brain.
